Your EDUCATION COMMITTEE, to whom has been referred

has had it under consideration and reports same with recommendation that it DO PASS, amended as follows:

1. On page 1, line 23, strike "complete" and insert in lieu thereof "be provided".

2. On page 1, lines 24 and 25, strike "that shall be forwarded to the human services department. Students shall be".

3. On page 2, line 1, strike "provided with human services department" and insert in lieu thereof "and".

4. On page 2, line 3, following the period insert "Schools shall forward to the human services department a list, complete with parents' names and addresses, of students who have been provided with such information.".

5. On page 2, line 11, strike "complete" and insert in lieu thereof "be provided with".

6. On page 2, line 12, after "eligibility" strike the remainder of the line, all of line 13 and on line 14 strike "be furnished" and insert in lieu thereof "and".

7. On page 2, line 16, following the period insert "The child-care facility shall forward to the human services department a list, complete with parents' names and addresses, of children who have been provided with such information.".
