    1. Strike House State Government, Elections and Indian Affairs Committee Amendments 1 through 6.

    2. On page 1, line 25, strike "total".

    3. On page 2, line 1, strike "prescription insulin drugs" and insert in lieu thereof "a preferred formulary prescription insulin drug or a medically necessary alternative".

    4. On page 2, line 2, strike "fifty dollars ($50.00)" and insert in lieu thereof "twenty-five dollars ($25.00)".

    5. On page 2, line 3, after "supply", insert a period and end quotation marks, strike the remainder of the line and strike all of lines 4 through 8.

    6. On page 3, line 6, strike "total".

    7. On page 3, line 7, strike "prescription insulin drugs" and insert in lieu thereof "a preferred formulary prescription insulin drug or a medically necessary alternative".

    8. On page 3, line 8, strike "fifty dollars ($50.00)" and insert in lieu thereof "twenty-five dollars ($25.00)".

    9. On page 3, line 8, after "supply", insert a period, strike the remainder of the line and strike all of lines 9 through 13.
