    2. On page 2, line 11, after "service", insert "to unserved and underserved areas as determined by the commission".

    3. On page 3, line 5, after the second occurrence of "a", insert "voice-enabled".

    4. On page 3, line 8, strike ". In prescribing" and insert in lieu thereof "as determined by the commission. Such surcharges shall be".

    5. On page 3, strike lines 10 through 12 in their entirety and strike line 13 up to the period.

    6. On page 4, line 16, after "services", insert "and areas".
