    1. On page 1, line 13, strike "DAILY" and insert in lieu thereof "MONTHLY".

    2. On page 1, line 22, strike "day" and insert in lieu thereof "month".

    3. On page 2, line 1, strike "daily" and insert in lieu thereof "monthly".

    4. On page 5, line 17, strike the brackets and remove the line through "monthly" and strike "daily".

    5. On page 7, line 16, strike "daily" and insert in lieu thereof "monthly".
