     1. On page 3, line 12, strike the word "and".

     2. On page 3, strike subparagraph (b) in its entirety.

     3. On page 10, line 14, after the word "state" insert a period and strike the remainder of the line; strike all of lines 15 and 16 and line 17 through the period.

     4. On page 11, line 14, after the word "state" insert a period and strike the remainder of the line, and all of lines 15 and 16 and line 17 through the period.
