    1. On page 7, line 15, strike "incompetence" and insert in lieu thereof "illness".

    2. On page 13, line 9, after "person" insert ", unless I have successfully challenged the determination of incapacity".

    3. On page 23, strike lines 13 through 15 in their entirety and insert in lieu thereof a closing quotation mark and period.

    4. On page 24, line 17, after "treatment" strike the comma and insert "or".

    5. On page 30, lines 23 and 24, strike "A principal appearing to require mental health treatment and lacking capacity shall be examined" and insert in lieu thereof "A determination of incapacity may only be made".
