1. On page 1, line 11, strike "DEATH", strike line 12 through "RECORDS" and insert in lieu thereof "DEATHS".

2. On page 1, line 12, strike the second hyphen.

3. On page 1, line 19, strike the second hyphen and strike "health".

4. On page 2, line 2, strike "licensed" and strike the second hyphen.

5. On page 2, line 8, after "study" insert "in accordance with adequate resources and".

6. On page 2, line 9, strike "licensed", strike line 10 through "extended" and insert in lieu thereof "long-term".
