1. On page 9, line 11, strike "government" and insert in lieu thereof "governmental institution or".

2. On page 9, line 12, strike "federally" and insert in lieu thereof "governmentally".

3. On page 14, line 22, following "a" strike "non-owner dentist" and insert in lieu thereof "non-dentist owner".
