DETERMINATION OF ELIGIBILITY FOR CHILD HEALTH | 0016| ASSISTANCE PURSUANT TO TITLE 21 OF THE SOCIAL SECURITY ACT-- | 0017| LOCATIONS OF ELIGIBILITY WORKERS.--Determinations of | 0018| eligibility for child health assistance pursuant to an | 0019| approved state plan under Title 21 of the Social Security Act | 0020| shall be made by state eligibility workers. To increase | 0021| accessibility for eligibility determinations, the workers | 0022| shall be located in: | 0023| A. local and district offices of the human | 0024| services department; | 0025| B. local and district offices of the department of | - 1 - 0001| health; | 0002| C. local and district offices of the children, | 0003| youth and families department; | 0004| D. hospitals, clinics and health centers providing | 0005| health services to children; and | 0006| E. locations that are accessible to potential | 0007| beneficiaries of child health assistance and appropriate for | 0008| placement of eligibility workers. | 0009|  |
