HOUSE BILL 434, as amended

 

has had it under consideration and reports same with recommendation that it DO PASS, amended as follows:

 

1. Strike Health and Human Services Committee Amendment 5.

 

     2. On page 6, line 25, strike "and".

 

     3. On page 7, line 6, strike the period and insert in lieu thereof "; and".

 

     4. On page 7, between lines 6 and 7, insert the following new paragraph:

 

                "(6) facilitation of meetings of providers who comprise the multidisciplinary team by means that ensure ongoing and consistent collaboration and coordination.".

 

     5. On page 9, line 12, after "DEPARTMENT", insert "OF HEALTH".

 

     6. On page 9, line 17, after the first and second occurrence of "department", insert "of health".

 

     7. On page 9, line 18, strike "may" and insert in lieu thereof "shall".

 

     8. On page 11, line 2, after "department", insert "of health".

 

     9. On page 11, line 3, after "department", insert "of health".
