    1. On page 1, line 18, strike "FIVE" and insert in lieu thereof "THREE".

    2. On page 2, line 18, strike "five" and insert in lieu thereof "three".

    3. On page 2, line 19, strike "files" and insert in lieu thereof "completes", after "application", insert "form" and strike "a".

    4. On page 2, line 20, strike "license" and insert in lieu thereof "licensure".

    5. On page 4, line 11, strike "five" and insert in lieu thereof "three".

    6. On page 4, line 12, strike "files" and insert in lieu thereof "completes", after "application", insert "form" and strike "a".
