    1. On page 1, line 12, strike "NOTICE" and insert in lieu thereof "INFORMATION ABOUT".

    2. On page 1, line 13, strike the first "OF".

    3. On page 1, line 18, strike "NOTICE" and insert in lieu thereof "INFORMATION".

    4. On page 1, line 19, strike "is entitled to notice of" and insert in lieu thereof "shall be provided with information about" and after "medicaid" insert "or general assistance".

    5. On page 1, line 22, strike "notice of expiration of benefits" and insert in lieu thereof "such information".

    6. On page 1, line 23, strike "a" and insert in lieu thereof "the" and strike "for".

    7. On page 1, line 24, strike "notice of expiration of benefits".

    8. On page 2, line 2, strike "data" and insert in lieu thereof "access".

    9. On page 2, line 3, strike "input".

    10. On pages 2 and 3, strike Section 2 in its entirety.,

and thence referred to the JUDICIARY COMMITTEE.
