    1. On page 1, line 21, after "performed" insert "by or,".

    2. On page 1, line 22, after "referral" strike "by" and insert in lieu thereof "of,".

    3. On page 1, lines 22 and 23, strike "licensed physician, nurse practitioner, certified nurse midwife or physician assistant" and insert in lieu thereof "health care provider, state agency or public or private health care organization's medical director or chief medical officer".
