has had it under consideration and reports same with recommendation that it DO PASS, amended as follows:

1. On page 1, lines 11 and 12, strike "IDENTIFICATION OR SWORN STATEMENT" and insert in lieu thereof "SIGNATURE".

2. On page 1, lines 23 through 25, strike the underscored language.

3. On page 2, lines 1 through 11, strike the underscored language,

and thence referred to the JUDICIARY COMMITTEE.
